Home and Community Care
Who benefits from Home and Community Care Services? Persons of any age who have had a nursing assessment and the nurse determines that there is a need for care. This may include:
•Persons who have been discharged from hospital. •Persons who have an illness or disease and need follow up care. •Persons who choose to live at home instead of a Personal Care Home or who are unable to live alone while waiting for care in a long-term care facility (Personal Care Home). •Persons who have a disability and require assistance to live on their own. •Persons who need nursing care in the home. Others who may benefit from Home and Community Care Services are: •Family and friends who need support to be able to continue to care for people in their home. How are people referred? •You may refer yourself or others may refer you, such as family members, friends, your doctor, neighbors, or hospitals. Funding •First Nation and Inuit Health Branch of Health Canada funds the Home & Community Care Program. •Funding that is provided to your community to deliver Home and Community Care services are based on services being provided from Monday to Friday from 8:30am – 4:30pm. Other Services •The Adult In-Home Care Program provides Homemaking services for eligible clients through your community’s Social Program and is funded through the Department of Indian Affairs and Northern Development. •Please contact your Home and Community Care Coordinator or Social Program Manager if you would like to know if you are eligible to receive services from this program.
